It’s appearing that a piece of insulation which knocked off some of the protective heat tiles at the launch of Columbia may be a contributing cause to the accident. When Columbia reentered Earth’s atmosphere, the damage was great enough to rip off even more tiles, causing the left wing to heat up to the point of structural collapse.
